Locations & Directions

Our main and branch office locations, contact number sna directions-interactive maps are available for each of our sites. All locations, except Puyallup clinical are open Monday through Friday until 5:00 PM; Kent opens at 8:00 AM and all branches open at 8:30 AM. Performance is closed on major Holidays. Puyallup clinical is open by appointment only.
