Our Services
We offer comprehensive respiratory and sleep therapy services designed to support patients confidently at home and complement your clinical care. Our full spectrum of therapies includes home mechanical ventilation (HMV), airway clearance, oxygen therapy (including portable concentrators), and CPAP and BiPAP therapy—supported by individualized education, setup, and follow-up.(Availability may vary by location).

Required Documentation: Sleep Therapy (PAP)
To refer a patient for PAP therapy, please include the following:
- Patient demographic information
- Standard Written Order (SWO) that includes:
- Patient name and order date
- General description of the item (HCPCS code or brand/model)
- Quantity to be dispensed
- Ordering practitioner’s name and NPI
- Qualifying sleep study (polysomnography or home sleep test)
- Chart notes from a face-to-face evaluation detailing:
- Reason for prescribing PAP therapy
- Diagnosis of OSA
- Planned course of treatment
- Signature meeting CMS guidelines
Medicare Sleep Study Criteria
Medicare-covered diagnostic sleep test scored at 4% and meets either:

Apnea-hypopnea index (AHI) or Respiratory Disturbance Index (RDI) ≥ 15 events per hour with a minimum of 30 events; OR

AHI or RDI ≥ 5 and ≤ 14 events per hour with minimum 10 events and documentation of:

Excessive daytime sleepiness, impaired cognition, mood disorders, or insomnia; orHypertension, ischemic heart disease, or history of stroke.

Documentation the beneficiary and/or their caregiver has received instruction fromthe supplier of the PAP device in the proper use and care of the equipment

Required Documentation: Oxygen Therapy
To refer a patient for oxygen therapy, please include the following:
- Patient demographic information
- Standard Written Order (SWO) with:
- Patient name and order date
- Description of oxygen equipment and supplies
- Quantity to be dispensed
- Ordering practitioner’s name and NPI
- Chart notes including:
- Prescription details (flow rate, delivery method)
- Clinical status at time of need
- Test results (Pulse Ox or ABG), test conditions (rest, exertion, sleep), and interpretation
- Reason therapy is expected to benefit the patient
- Standard Written Order (SWO) with:

- Description of oxygen equipment and supplies
- Quantity to be dispensed
- Ordering practitioner’s name and NPI

Qualifying Saturation Criteria
- Group I: SpO2 ≤ 88% or PO2 ≤ 55 mm Hg
- Group II: SpO2 = 89% or PO2 = 56–59 mm Hg with one of the following:
- Dependent edema
- Pulmonary hypertension or cor pulmonale
- Hematocrit > 56%
- Group III: Conditions such as cluster headaches or documented variations requiring oxygen therapy
