Once you receive the link, click on it, and follow the instructions to complete your virtual mask fitting. Once you have finished your scan, the results will be available to us.
If you are on service for your equipment and supplies with a different medical equipment provider, you will receive a text message with a form to allow us to transition your service to Performance Home Medical.
After our team completes your verification and authorization, you will receive your benefits packet via email, which includes your insurance plan’s specific coverage information, your deductible/co-pay information, and instructions to complete the documentation.
